Memorial Hermann Sports Medicine & Rehabilitation Greenway Plaza/Upper Kirby offers a wide array of services from general therapy to specialty programs, with highly skilled and certified therapists. As part of patient-centered care, therapists perform comprehensive evaluations to determine appropriate treatment to individually address your specific goals.
Please note that all patients under the age of 16 are required to be accompanied by an adult, adult family member or legal guardian during their treatment time.
Patients 16 years of age or older are required to be accompanied by an adult during their initial appointment for paper work and consent purposes. After the initial visit, adult supervision may no longer be required.
If an adult is not present with the patient as listed by our age requirements, the patient may be asked to be rescheduled.
Maria Fernandez Cuadrado, PT, DPT has a doctorate in physical therapy and has completed advanced training in differential diagnosis. She is board certified in physical therapy and certified in basic life support. She specializes in prenatal and postnatal therapy as well as therapy for pelvic pain, stress urinary incontinence (SUI), mixed urinary incontinence (MUI), bladder and bowel control, constipation, pelvic girdle pain (PGP) and sacroiliac joint dysfunction. Maria sees patients ages 18 and older. She is available for early morning and evening appointments in addition to regular business hours.
Qui Le, PT, DPT, OCS, SCS has a bachelor’s degree and doctorate in physical therapy, and has completed a fellowship program and advanced training in differential diagnosis. He holds board certifications as both an orthopedic specialist and a sports specialist. He specializes in orthopedic and sports therapy, focusing on the shoulder, hip, knee, and foot/ankle for patients ages 12 and older.
Danie Nguyen, PT has a Bachelor of Science in physical therapy and has completed advanced training in differential diagnosis. In practice since 1986, she provides McKenzie Method and functional exercises for neck/shoulder, knee, ankle and foot conditions. Danie sees adolescent through senior patients and is available to schedule early morning appointments in addition to regular clinic hours.
Colette Pientok, PT, DPT graduated from the University of Wisconsin with a Bachelor of Science and completed her Clinical Doctorate in Physical Therapy through the College of St. Scholastica. Colette has completed advanced training in manual therapy, functional exercises, and differential diagnosis. She utilizes a variety of treatment approaches including McKenzie Method, Mulligan, and IASTM. Colette’s areas of practice include spine, upper and lower extremities for patients ages 8 and older.
Monica Shroff, DPT has a Bachelor of Science in kinesiology and a doctorate in physical therapy. Certified in mechanical diagnosis and therapy (McKenzie Method), she specializes in lumbar and cervical spine pain (with or without radiation); hip, knee and spine pain in runners; and management of musculoskeletal pain in patients with neurological history. Monica, who has been in practice since 2010, sees patients ages 8 and older. She is available to schedule early morning and evening appointments in addition to regular clinic hours.
Frank Ward, DPT has a bachelor’s degree and doctorate in physical therapy, and has completed a fellowship program and advanced training in differential diagnosis. He is board certified in orthopedics, a certified Mulligan practitioner, a diplomat in the McKenzie Method of mechanical diagnosis and therapy and a fellow of the American Academy of Orthopedic Manual Physical Therapists. In practice since 1991, he specializes in conservative management of spinal conditions, differential diagnosis and management of complex lumbo-pelvic-hip disorders and shoulder adhesive capsulitis. Frank sees patients ages 12 and older and is available to schedule early morning and evening appointments outside of regular clinic hours.
Gail Zitterkopf, PT, DPT, CLT specializes in the evaluation and treatment of pelvic floor, prenatal and post-partum conditions with a focus in gynecological oncology as well as gender confirming. She also has experience dealing in congenital conditions such as vesicoureteral reflux, constipation and nocturia in children. Her approach to physical therapy is to develop care plans that align with the patient’s goals. She has been Memorial Hermann’s Pelvic Floor Program Coordinator since 2013, and is certified in Pelvic Muscle Dysfunction Biofeedback through the Biofeedback Certification Institute of America. She is also a Certified Lymphedema Therapist through the Norton School of Lymphatic Therapy.
Surface parking is located in front of our suite. Patients can also park in the bank or Costco area. There are limited Memorial Hermann Reserved spaces in front of the clinic. There is additional garage parking with Memorial Hermann spaces on the west end of the 3rd floor or the garage. When parking in the garage you are not required to park in the Memorial Hermann spots. There is only a breezeway connection to the front of the center from the 3rd floor.
If parking in the garage, you will walk through the breezeway next to Buffalo Wild Wings. There is an elevator on the Weslayan side to the north of the stairs. You may also take the stairs to the first floor. If you park on the lower level of the garage, you will need to walk around the building to get to the entrance of our suite.
