Memorial Hermann Community Benefit Programs are designed to provide care for uninsured and underinsured; to reach those Houstonians needing low-cost care; to support the existing infrastructure of non-profit clinics and FQHCs; to close the mental health gap in mental and behavioral care services; and to educate individuals and their families on how to access the services needed by and available to them. Memorial Hermann Health System conducts a Community Health Needs Assessment for each of its facilities in accordance to the Patient Protection and Affordable Care Act to identify the community’s health needs and strengths to prioritize health issues, provide a foundation for the development of a community health improvement plan, and assist community organizations in their program planning.
Most recently completed in June 2019 with the intent to identify the major health care issues, barriers to access, and health priorities for those living in the Houston community, the analysis included a review of secondary data on social, economic and health issues in the region with qualitative information from focus groups with community residents and service providers and input from numerous community representatives.
The resulting Strategic Implementation Plans (SIPs) set hospital-specific goals, objectives and strategies that address each facility’s specific service area and populations served. CHNAs and SIPs are available for the following facilities:
